The Document Portal will be available beginning July 2, 2018. Below please find training videos which provide directions on the newly enacted document exchange for requests of eligibility or accommodations on behalf of student-athletes. The document portal is required for exchange of the following forms:
- Athletic Transfer Waiver Request
- Request for Decision of Athletic Eligibility
- Requests for Modifications of the Uniform Requirements
- Requests for Use of a Motorized Golf Cart
- Requests for Accommodations Due to Disabilities
